Notice of the Purpose of Using Personal Information and Disclosure of Personal
Information
You are responsible for any mailing costs when you mail us an inquiry form.
Please use delivery-certified mail or other similar services that confirm delivery.
735 yen is charged to you as an administrative fee for each disclosure and
purpose of use inquiry (tax included). Please include postage or a postal money
order* of 735 yen with your inquiry form when you request the notice of the
purpose of using personal information or the disclosure of personal information.
We will contact you if the amount is not sufficient. If we do not receive the
fee in one week after contacting you, we consider that there is no inquiry.
* "Postal money orders" are issued at post offices. You are responsible
for any issued cost when you are issued Postal money order. When requesting
the notice of purpose of using personal information and the disclosure of personal
information in one inquiry, you only need to cover one fee (735 yen). Once
receiving a request, we do not refund the fund even when we do not retain your
personal information or we cannot disclose due to reasons defined under decree.
［II. Confirmation of the Identity of a Principal or an Agent］
We make a confirmation by phone as soon as we receive an inquiry form by mail.
We ask you or your agent some questions such as your address and birth date in
order to confirm the identity of a principal or an agent.
［III. Correction］
We make a correction, upon confirming your personal information, by inquiry forms.
The final data after correcting personal information (including when we cannot
make any correction) is mailed to the principal's address, confirmed by inquiry
forms and identity confirmation documents, by delivery-certified mail.
［IV. Removal of Personal Information, Discontinuation of Use and Discontinuation
of Provision to Third Parties］
When you remove personal information, discontinue the use of personal information,
and/or discontinue the provision of personal information to third parties, you
may not be able to receive services that you are currently using.
Even if we receive a request for the removal of personal information, discontinuation
of use and/or discontinuation of the provision of personal information to third
parties, please note that we may not be able to make a change to a part of personal
information if we retain your credits or in other circumstances.
［V. Others］
We use your personal information written in an inquiry form within an appropriate
range in order to make a correction.
We do not return inquiry forms submitted to us. All the forms are securely managed
at our companies.
We may contact you or your agent by phone in order to confirm the contents of
an inquiry form. Procedures regarding correction may be changed partially or
as a whole without any notice. Please confirm with us when you request any correction
to your personal information.
